Employee Benefits at a Glance

Medical Insurance

At NSU, you can choose between three different PPO medical plans from ICUBA, administered by BlueCross BlueShield. Prescription Pharmacy Benefits from OptumRx and Behavioral Health Benefits from Aetna accompany your medical insurance election but are administered separately. NSU also provides a Health Reimbursement Account (HRA), accessible using the ICUBA MasterCard, to each employee enrolled in an NSU/ICUBA Medical Insurance plan!

HRA funds can be used to offset out-of-pocket costs for eligible medical, dental, and vision care expenses for covered members. HRA contributions begin at the end of your second month of participation in an ICUBA medical insurance plan and continue as long as you remain enrolled!

Medical Insurance from BlueCross BlueShield will be updated at the close of the business day following your new hire enrollment or the approval of the life event by the Office of Human Resources.
Example: A life event approved on Friday will be updated with BlueCross BlueShield before the end of the following Monday and your benefit election will be active in their system on Tuesday morning (actual effective dates of coverage are based on the life event documentation submitted).

Surgery Plus+

When eligible procedures are scheduled through a SurgeryPlus+ Concierge Care Specialist, you don't pay!  You read that right...no deductible and no out-of-pocket maximum apply. You may even be eligible for travel reimbursement if you elect to use a surgeon more than 50 miles from your home. 

SurgeryPlus+ is available to all employees and their dependents who are enrolled in a medical insurance plan at NSU through ICUBA. If you are enrolled in medical insurance, you do not have to mak an election for SurgerPlus+, you will have automatic access to their concierge service for non-emergency, elective procedures like knee and hip replacement surgeries. 

All procedures must be scheduled directly with a Surgery Plus+ Concierge Care Specialist who will provide you with a list of 3-5 surgeons to choose from.

GAP Insurance (APL)

Gap insurance can help offset medical expenses you may incur before you meet your deductible and out-of-pocket maximum for the new plan year.

In order to enroll in Gap insurance during Open Enrollment, you must be simultaneously covered under an NSU medical insurance plan at NSU. Gap insurance plans have the same tiers as the NSU medical insurance plans - employee only, employee+child(ren), employee+spouse, and family.

GAP Insurance (APL)
Prescription Pharmacy Benefits

Prescription Pharmacy Benefits accompany your Medical Insurance election but are administered separately by OptumRx. Be sure to bring your OptumRx card when going to a new pharmacy, prescription benefits cannot be verified with your BlueCross BlueShield card. 

If you are covered under an NSU medical insurance plan you can fill your preferred generic prescriptions at the NSU Clinic Pharmacy at no cost! The NSU Clinic Pharmacy is located at 3200 S. University Drive on our Fort Lauderdale campus. For more information call the NSU Clinic Pharmacy at 954-262-4550.

Employees and their family members covered under an ICUBA medical insurance plan also have access to the ICUBAcares Pharmacist Advocate at no cost!  The ICUBAcares Pharmacist can help with prior authorizations, step therapy, and much more. Flexible Spending Accounts

Health Care Flexible Spending (HCFSA) and Dependent Care Flexible Spending (DCFSA) accounts are funds that you set aside from your paycheck before taxes are taken out. You can use your pre-tax flexible spending account dollars to pay for eligible expenses throughout the plan year for you, your spouse, and your eligible dependents. The Flexible Spending Account Benefit Plan year begins - April 1, 2018 and ends - March 31, 2019. You can Incur expenses until June 15, 2019 and have until June 30, 2019 to file any unclaimed expenses. 

How do HCFSA elections work?

You make an annual healthcare flex spending election that is deducted from your paycheck through the benefit plan year. Your HCFSA funds are advanced once your make an election and you have access to those funds for immediate use for eligible medical expenses. 

How do DCFSA elections work?

You make an annual dependent care flex spending election that is deducted from your paycheck through the benefit plan year. Your DCFSA funds are made available each pay period when the deduction is taken from your paycheck and can be used for eligible dependent care expenses that you have already incurred. You cannot prepay for services with your available DCFSA funds. 

FSA and HRA Eligible Expenses

Dental Insurance

At NSU, you can choose between three different Humana Dental insurance plans from ICUBA.

The Dental HMO (CS250) plan is only available in Florida and has copays for scheduled services. Selecting an in-network dentist is required when enrolling in the DHMO plan. 

The Preventive Plus PPO plan provides Preventive and basic services only with no network restrictions

The PPO High Dental Coverage Plan provides preventive, basic, & major services as well as coverage for adult and pediatric orthodontia with no network restrictions.

Vision Insurance

At NSU, there are two EyeMed Vision Insurance plans available from ICUBA. 

The Base Vision plan covers vision exams every 12 months, frames every 24 months or contact lenses every 12 months with a $100 retail allowance. 

The Enhanced (Buy-Up) Vision plan covers vision exams every 12 months, frames every 12 months or contact lenses every 12 months with a $130 retail allowance.

Behavioral Health Benefits

Behavioral Health & Substance Abuse benefits accompany your Medical Insurance election but are administered separately by Aetna.  Be sure to bring your Aetna card when going to a new provider, Behavioral Health & Substance Abuse benefits cannot be verified with your BlueCross BlueShield card. Aetna, Resources for Living also administers the Employee Assistance Program (EAP), free for all NSU employees!

Pet Assure with Pet Plus Rx

You asked and we listened!

Effective April 1, 2019, employees will be able to elect the Pet Assure with Pet Plus Rx benefit from Pet Benefit Solutions for single and multiple pet households.

Pet Assure provides discounts on all in-house medical services with no deductible, no exclusions, and no waiting period. Pet Plus prescription coverage provides access to reduced cost prescription medication with free shipping or pick up at more than 50,000 pharmacies. Coverage is available for single and multiple pet households.

Aflac Personal Insurance

Aflac coverage is available to complement your current benefits and fill gaps in coverage.  Full policy benefits are payable regardless of other coverage at NSU.  Aflac coverage is available for short term disability, hospitalization, accidents, and cancer.

Retirement Savings

When do you get the Match?

Employees who are eligible, working 19.2 hours or more per week, have attained age 21, and have completed one year of service at Nova Southeastern University are eligible to receive basic and  matched contributions in the 401(k) Safe Harbor Retirement Savings Plan at Nova Southeastern University. Eligible employees can enroll on the first of the month following their one-year employment anniversary.

Savings  are tax deferred until you take a withdrawal and you are not taxed on the funds until then!

Annual contribution limits for 2018 are $18,500, or $24,500 if you are age 50 or older.

Valic and TIAA are NSU's retirement plan vendors.

Employees at NSU can maximize retirement savings by contributing on a non-matched (supplemental) basis as early as the first of the month following their date of hire!

Life and Disability Insurance

NSU's Short Term Disability Program

The Short Term Disability program is sponsored by NSU and designed to provide reduced salary for eligible employees who are disabled for seven (7) or more days and unable to perform their job.  This benefit runs concurrent to Family Medical Leave for eligible employees and requires employees to have exhausted any available paid time off prior to receiving benefits under the program.

Long Term Disability Insurance

Long Term Disability is an insurance product designed to provide reduced salary benefits to eligible participants who become totally disabled under the terms and conditions of the Program.

Basic Life and Accidental Death Insurance

Basic Life Insurance is the equivalent of an employee’s annual base salary (does not include overtime or overload).

Optional Life Insurance 

Group Term Life Insurance is available from Reliance Standard. Employees are eligible after 90 days of employment when working 19.2 hours or more per week in an eligible position.

Life and Disability Insurance
Tuition Waiver and Reduction

Tuition waiver benefits are available for all eligible employees who have successfully completed six months of continuous employment at NSU. Tuition reduction benefits are available to qualifying family members of eligible employees who have successfully completed six months of continuous employment and to certain vendor employees who are contracted through the Business Services or Facilities Management Department.

Don't Forget!

The Benefit Plan Year at NSU begins on April 1 and ends on March 31 the following year, deductible and out-of-pocket limits are reset annually on April 1.

New elections or election changes for medical, dental, vision, flexible spending, optional life insurance, Aflac, and LegalShield are made through the ICUBA Enrollment Portal at http://icubabenefits.org.

New employees must make elections or waive coverage within 30 days of their date of hire. Current employees experiencing a qualifying status change (life event) must request changes within 30 days of the event and upload documentation to their Employee File to support the change in status.

Dependent verification is required for dependent children, spouses, or domestic partners when being added to employee benefits through NSU/ICUBA.
