Note: Vision insurance from EyeMed will be updated on Thursday following your new hire enrollment or the approval of the life event by the Office of Human Resources (e.g. A life event approved on Friday will be updated with EyeMed before the close of business on the following Thursday and your benefit election will be active in their system on Friday morning [actual effective dates of coverage are based on the life event documentation submitted]).
Eye Med Plans:
- Annual eye exams ($5 copay)
- Single Vision Lenses ($15 copay)
- Frames every 24 months ($0 Copay + $130 allowance; 20% off balance on frames over $130)
- Contact lenses every 12 months ($100 allowance or balance left on contact lenses over $100)
2024-2025 Base Plan Premiums
- Employee Only: $4.74 monthly / $2.37 biweekly
- Employee + Family: $12.15 monthly / $6.08 biweekly
