Form 1095-B is a health insurance tax form used to report information to the Internal Revenue Services (IRS) and to taxpayers.  Information reported includes type of coverage, dependents covered under the policy, period of coverage, and more.

Form 1095-B is mailed to all students covered under the NSU Student Health Insurance Plan by the insurance company at the beginning of each calendar year. For more details about Form 1095-B, visit the IRS website.

NSU is required by law to report receipt of more than $10,000 in cash over a course of a calendar year to the IRS by filing an IRS Form 8300. Money orders, cashier's checks, and traveler's checks are considered cash payments.

Personal and corporate checks are not considered cash payments. Students will be notified by the Bursar's Office once a Form 8300 has been filed.
