Whether you're a new or returning patient, this page provides all the information you need for appointments, forms, payments, and prescription refills to make your visit as smooth as possible.

Appointments

Please plan to arrive 30 minutes before your appointment to allow for time to complete any necessary paperwork.

Please bring the following with you to every appointment:

  • A photo ID
  • Payment (We accept checks, American Express, Visa, and Mastercard)
  • A list of symptoms or problems you have been experiencing including how long you have had it/them and how you are affected
  • A list of current prescription and non-prescription medications, vitamins, dietary supplements, or herbs you are taking including frequency and dosage
  • Questions for your physicians
  • Medical records from your previous provider

If you see other physicians in between your visits, bring information from those appointments with you, including a list of prescription medications, test results, and prescribed treatments.

Please plan to spend about two hours with the physician for your initial visit, and one and a half hours for return visits. During your appointment, diagnostic and lab tests may be performed.

To make the most of your time, come prepared with a list of questions like:

  • What is my main problem?
  • What should I do next?
  • Why is this treatment important?

We also suggest that you:

  • Gather information about your family's medical history.
  • Get laboratory tests or procedures done in advance so your physician has the results.
  • Bring a family or friend for support and to ask questions, if needed.

Your input is crucial. Don't hesitate to share your opinions, concerns, or anxieties about your condition or treatment. Your doctor will use this information to tailor your care plan.

The INIM doesn't accept private insurance. The INIM operates as a self-pay clinic, and we accept checks, Visa, MasterCard, and American Express. A $25 fee will apply to returned checks. 

As a specialized clinic, our fees reflect the expertise of our health care providers and the time spent with each patient. Payment is due at the time of service, and we encourage patients to check with their insurance companies about reimbursement options.

 If you need to cancel an appointment, please notify us at least 24 hours in advance. 

Before your first appointment, we ask that you track your blood pressure and pulse for seven days and bring the records to your appointment.

Here’s how to take your readings:

  • First thing in the morning, while lying down
  • 10 minutes after standing up and moving around
  • During any “crash” periods throughout the day

Complete the New Patient Form and Online Consent Forms before your first visit.

To provide you with the highest standard of care, all returning patients must complete the following forms before each visit:

Please bring a list of any medications, including over-the-counter supplements, to your appointment. 

For refills, please call (954) NSU-CARE (678-2273). Be sure to include your pharmacy's contact information and allow three to four business days for processing. To avoid delays, we recommend requesting refills before you run out of your medication. Some medication requests will require a prior authorization from your insurance company or may require a visit to our office.

New patients must complete a Medical Release Form to transfer records from previous physicians.

If you need prior medical records or test results, let our staff know where to request them. Please note that we will need your written authorization for each facility or provider sending medical records to us.

The INIM does not provide after-hours care. For emergencies, dial 911 and follow up with your doctor during regular business hours.
