Students who receive Title IV funds as part of their financial aid package should complete this form which will authorize the Office of Student Financial Assistance to apply current available Title IV funds to current year non-institutional charges.

Title IV funds include the following: Federal Direct Loans (Subsidized, Unsubsidized, Parent PLUS, and Grad PLUS), Perkins Loan, Federal Pell Grant; Federal Supplemental Educational Opportunity Grant (FSEOG), and Teacher Education Assistance for College and Higher Education (TEACH) Grant.

Nova Southeastern University (NSU) will primarily use a student’s Title IV funds to credit a student’s account for institutional charges such as tuition, fees, and room and board (if the student contracts with the school). NSU may also apply a student’s Title IV federal financial aid funds toward other non-institutional charges if NSU obtains authorization from the student. Non-institutional charges may include application fees, campus card replacement fees, transcript fees, and late fees.

1. Authorization to Apply Title IV federal financial aid funds

I authorize NSU to apply my Title IV federal financial aid funds to pay for my other non-institutional charges posted to my student account.

2. Prior Year Charges

NSU is not required to obtain a student's authorization to apply current financial aid funds to prior academic year charges. However, please note that NSU can only apply a maximum of $200 of Title IV federal financial aid funds toward prior academic year charges.

I have read and understand the information contained in this Authorization Form, and agree it is accurate and complete. This authorization shall remain in effect throughout my enrollment at NSU unless I modify or cancel this authorization by completing and submitting a new authorization form to the Office of Student Financial Assistance.
