Billing for Florida Prepaid College Plans will occur after the add/drop period has concluded. Florida Prepaid can cover tuition, fees, and costs for on-campus housing based on the student's hours of enrollment after the drop/add period. Use this form to indicate your Florida Prepaid Plan billing preferences. Be sure to contact Florida Prepaid at 800-552-4723 to provide them with the same instructions.

Restricted Plan
I request that NSU limit the amount to be billed to my Florida Prepaid College Plan each semester to the number of credit hours I am registered for each semester after the drop/add period. I understand that the value of each credit hour is subject to change at the beginning of each academic year as determined by Florida Prepaid.

Unrestricted Plan (Students with this plan may qualify for funds exceeding hours of enrollment.)

By submitting this form, I understand and agree that the NSU Office of the University Bursar will bill my Florida Prepaid Plan as instructed on this form until (1) my funds are exhausted or (2) I submit a new Florida Prepaid College Plan Billing Request Form prior to the start of each semester to request changes.
