NSU is able to offer the marketplace equivalent of gold level healthcare coverage that provides exceptional value. Employees and their families enrolled in healthcare coverage at NSU can continue to rely on the affordability, stability, and high quality of benefits available through our continued relationship with ICUBA.

ICUBA & Optum Rx have improved the member experience with a designated Health Care Advisor service team and new customer service phone number.

The Optum Health Care Advisor replaces the standard pharmacy customer service for all member calls and is designed to help members to efficiently utilize their prescription benefits.

Behavioral Health & Substance Abuse Coverage Provided by Aetna

Behavioral Health & Substance Abuse coverage is attached to health care and administered by Aetna.

Enrollment in Behavioral Health & Substance Abuse coverage is automatic when selecting a health care plan at NSU, no additional enrollment is required.

Aetna administers our Behavioral Health & Substance Abuse coverage and the Employee Assistance Program (EAP).

For employees enrolled in health care coverage, the transition from EAP utilization to an in-network Aetna provider is possible when using the Behavioral Health & Substance Abuse coverage after EAP services.

  • No cost to visit Primary Care Blue Distinction Total Care (BDTC) Physicians
  • No cost for Annual Wellness exams
  • No cost for routine labs at Quest Diagnostics
  • No cost for in-network maternity visits after the initial visit co-pay*

In addition to the benefits covered at 100%, NO DEDUCTIBLE is applied before the following services are covered under the plan:

  • No deductible before prescription co-pays apply
  • No deductible before in-network primary care co-pays or coinsurance apply
  • No deductible before PT/OT, and chiropractic visit co-pays or coinsurance apply
  • No deductible before Retail/Urgent Care visit co-pays apply

NSU provides employees Enrolled in Health Care with a Health Reimbursement Arrangement account 

The Health Reimbursement Arrangement or HRA account is provided to each employee enrolled in the ICUBA health care plan at NSU. An HRA can be used to offset out-of-pocket costs for certain medical, dental, and vision care expenses for covered members. HRA contributions begin at the end of your second month of participation in the ICUBA health care plan and continue every month you are enrolled. HRA account deposits: 

  • Do not expire, they rollover from month to month and year to year
  • Can be used for expenses for employees and family members covered on the NSU/ICUBA health care plan
  • Are administered using the ICUBA MasterCard (the same card for flexible spending)
  • Is vested and portable after 36 consecutive months of participation in an NSU/ICUBA health care plan
  • Can be used to pay for COBRA or retiree medical premium after separation (if vested at time of separation)

File claims, check balances, and report lost or stolen ICUBA MasterCards online at using the single sign on feature.

Flexible Spending Accounts can Help Minimize Out-Of-Pocket Expenses

Health Care Flexible Spending (HCFSA) and Dependent Care Flexible Spending (DCFSA) accounts are voluntary elections contributed to by employees based on annual elections.

Flexible Spending is a pre-tax deduction and offsets taxable income and is subject to the "use-it-or-lose-it" rule.

A full benefit year for Flexible Spending begins on April 1 and ends March 31 of the following year. Employees can continue to incur expenses on flexible spending elections until June 15 and must file any unclaimed expenses by June 30.

Flexible Spending benefit year example:

  • Year begins - April 1, 2017
  • Year ends - March 31, 2018
  • Incur expenses - June 15, 2018
  • File unclaimed expenses - June 30, 2018

Dental HMO (DHMO CS250) Plan

  • The dental HMO plan is available only in Florida
  • Provides co-pays for scheduled services
  • HMO network dentist selection is required at enrollment and prior to seeking treatment

There are two vision plans available to NSU employees through EyeMed.  

Base Vision Plan

  • Vision exams every 12 months

  • $100 frame or contact allowance

Enhanced (Buy-Up) Vision Plan

  • Vision exams every 12 months
  • Frames or contact lenses every 12 months
  • $130 frame or contact lens allowance

If you're eligible for Medicare here are some resources to help you navigate through the process.

Employees choosing to remain on the NSU/ICUBA plan once they are eligible for Medicare Part B will not receive a penalty for enrolling after age 65. The NSU/ICUBA plan may provide a greater level of coverage at a more affordable cost, especially when the Health Reimbursement Arrangement (HRA) provided by NSU is factored in to the cost of coverage.

Medicare comes in four parts: A, B, C, and D.  Medicare parts A and B are provided through the government and they are often referred to as “Original Medicare.” Together, they cover some of the basics like hospital stays and doctor visits. Most people need more coverage for things not covered by parts A and B, like prescription drug benefits.  A summary of each part of Medicare and links to additional resources are provided below.

Medicare Part A is a fundamental component of Medicare. It covers services like hospital stays and hospice care and is free with enrollment when turning 65. Enrolling in Medicare Part A does not require enrollment in Part B as long as you are eligible and remain covered in the NSU/ICUBA health care plan.  Enrollment in Medicare Part A is usually at no additional cost to eligible participants, for more information contact your local Social Security Office directly.

Medicare Part B pays 80% of approved charges for services like doctor visits and outpatient care. Enrolling in Part B is not required if you are eligible and remain covered under the NSU/ICUBA plan.  

Medicare Part C, called Medicare Advantage, typically bundle parts A, B, and D but you will still need to add benefits like dental, vision, and hearing.

Medicare Part D is Medicare’s prescription drug program and an option most people choose to add to their Medicare coverage.

SHINE (Serving Health Insurance Needs of Elders) is a free program offered by the Florida Department of Elder Affairs and your local Area Agency on Aging. Specially trained volunteers can assist you with your Medicare, Medicaid, and health insurance questions by providing one-on-one counseling and information. SHINE services are free, unbiased, and confidential.

Don't Forget!

The Benefit Plan Year at NSU begins on April 1 and ends on March 31 the following year, deductible and out-of-pocket limits are reset annually on April 1.

The current Benefit Plan Year ends March 31, 2017.

New elections or election changes for medical, dental, vision, flexible spending, optional life insurance, Aflac, and LegalShield are made through the ICUBA Enrollment Portal at

New employees must make elections or waive coverage within 30 days of their date of hire. Current employees experiencing a qualifying status change (life event) must request changes within 30 days of the event and upload documentation to their Employee File to support the change in status.

Dependent verification is required for dependent children, spouses, or domestic partners when being added to employee benefits through NSU/ICUBA.
