Page 39 • The right to request the amendment of the student’s education record that the eligible student or minor student's parent(s) believe is inaccurate or misleading. An eligible student or minor student’s parent(s) who believe that the student’s education records contain information that is inaccurate or misleading, or is otherwise in violation of the student’s privacy, may discuss such concerns informally with the division director. If the decision is in agreement with the request for amendment, the appropriate records will be amended. If not, the eligible student or minor student’s parent(s) will be notified within a reasonable period of time that the records will not be amended and will be informed by the NSU University School Registrar of the right to a formal hearing. • The right to consent to disclosures of personally identifiable information contained in the student’s education records, except to the extent that FERPA authorizes disclosure without consent. • The right to file a complaint with the Family Policy Compliance Office, U.S. Department of Education, 400 Maryland Avenue, SW, Washington, D.C., 20202-4605 concerning alleged failures by NSU University School to comply with the requirements of FERPA. DIRECTORY INFORMATION Nova Southeastern University hereby designates the following student information as public or directory information. Such information may be disclosed by the institution for any purpose, at its discretion. • student name • local and home address • telephone numbers • anticipated graduation date • email address An eligible student or minor student’s parent(s) may withhold directory information (as defined above) by logging into the My Backpack system and checking the “Do Not Publish” box for any given school year. RELEASE OF PERSONALLY IDENTIFIABLE INFORMATION An eligible student or minor student’s parent(s) can give consent to permit NSU University School to discuss and/or release personally identifiable information to a third party. This consent must be provided in writing with the signature of the eligible student minor student's parent(s). To provide written consent, complete an Authorization for Release of Information Form available on the NSU University School website at (uschool.nova.edu) . Personally identifiable information from the education records of a student may be released to the following third parties without written consent: 1. other school officials determined by the director or head of school to have legitimate educational interests in the student. A school official is a person employed by the university in an administrative, supervisory, academic, research, or support staff position (including law enforcement unit personnel and health staff); a person or company with whom the university has contracted (such as an attorney, auditor, collection agent or assisting another school official in performing his or her tasks). 2. officials of another school in which the student seeks or intends to enroll, provided the agency or institution transferring the records • makes a reasonable attempt to notify the parents or the eligible student, or includes a notice in its policies and procedures that it forwards education records on request. • provides the parents or eligible student, upon request with a copy of the records being transferred. • provides the parent or eligible student, upon request, with an opportunity for a hearing. Official student transcripts can only be sent to a third party and cannot be given directly to a student or parent. Only unofficial transcripts can be given to a student or a parent.
