Release of Personally Identifiable Information An eligible student or minor student’s parent(s) can give consent to permit NSU University School to discuss and/or release personally identifiable information to a third party. This consent must be provided in writing with the signature of the eligible student minor student's parent(s). To provide written consent, complete an Authorization for Release of Information Form available on the NSU University School website at (uschool.nova.edu) . Personally identifiable information from the education records of a student may be released to the following third parties without written consent: 1. other school officials determined by the director or Head of School to have legitimate educational interests in the student.
