Parent acknowledgment that, if the student does not observe school rules, and at the discretion of the chaperone, the student may be sent home at the parent's expense or the parent may be required to pick the student up from the field trip location. e. Parent’s acknowledgement that they must pick up students within 15 minutes of the return trip. 1. A telephone chain shall be prepared and duplicated so that one long distance call could be made to report an emergency change of plans. One copy of this list shall be kept with the permission slips in the school office. Each trip chaperone and each participant's parent/guardian shall also receive a copy. 2. A medical form for students with special needs shall be obtained and copies shall be kept by the teacher-in-charge and with the permission slips. PART 3: INSTRUCTIONAL PERFORMANCE AND PROCEDURES FACULTY/ STAFF EVALUATION The supervision and evaluation of faculty and staff is intended to: 1. Improve the instructional programs and effectiveness of supporting the mission of the school. 2. Assist in the professional development of teacher/staff, which is at the discretion of the administration, who may assign required experiences, require attendance at conferences or in specific classes, workshops or seminars, or require completion of a prescriptive set of learning experiences to improve or enhance performance. 3. Provide information needed to make employment decisions The evaluation system involves the supervision evaluation, and professional development of faculty and staff. The Director has primary responsibility for this evaluation and will elicit input from multiple venues and/or colleagues in the determination of the appraisal. An annual evaluation of performance is completed for each employee each year (see Appendix). INSTRUCTIONAL EXPECTATIONS NSU University School teachers are expected to: 4. Provide educational experiences for students that are consistent with NSU University School mission, vision and philosophy, values and goals. 5. Personalize the education for each student by applying effective and current research-verified instructional strategies to differentiate instruction within their fields, subjects and disciplines, maintaining attention to current advances in those subject areas.
