29 l. Plan for postponement or cancellation of the trip m. Space for parent/guardian signed consent 7. Plan and write itinerary. 8. Announce objectives and details of field trip to age appropriate students and work with other teachers to provide for involvement of students in the preparation for the trip. 9. Collect permission slips and money. 10. Group students with chaperones. Make bus lists. Provide the associate director with the permission slips and a copy of the master list of students and chaperones. Have a copy of each student’s medical information. On the day of the trip, leave an updated list with the administrator and nurse. 11. Ensure that students with specific health needs or physical limitations have whatever equipment/medicine they may need with them. When necessary the team may determine that these students are accompanied on the field trip by their parent/guardian. 12. Inform all chaperones of: a. The procedures to be followed in the event of an emergency, illness, or accident. b. The itinerary. c. Suggested questions/activities for achieving the objectives to be used with students. d. Specific duties for proper supervision. e. Suggested appropriate attire. f. Any special needs of a student in their group. 13. Prepare follow-up activity. 14. Confirm the reservations. 15. Inform the students of the itinerary, rules policy, and suggested appropriate attire. Remind them the day before. 16. Ensure that the chaperones have all the necessary information on the day of the trip. Provide each with a list of names. 17. Maintain a master list of students and chaperones assigned to each conveyance. Ensure that roll call of all persons is taken at the initial loading and each reloading during the trip. 18. Provide for supervision until the last student goes home as prearranged by the parent/guardian. 19. Report any mishaps to the administration upon return to school. 20. Evaluate objectives and success of achievement upon completion of the trip. B. Additional Requirements for Extended and Overnight Field Trips - In addition to the required transportation information, the following shall be included on or with the permission slip: a. Detailed itinerary b. Special clothing or cash needs c. Specific nighttime procedure
