If, at any time, information received in connection with a background check indicates that a student has provided false or misleading statements, has omitted required information, or in any way is unable to meet the requirements for completion of the program, then the student’s admission may be denied or rescinded, the student may be disciplined or dismissed, or the student’s enrollment may be terminated. Acceptance to a health college program does not guarantee that students with information of a concern will be accepted by clinical training facilities to which they may be assigned. If a student is unable to obtain placement at a clinical training facility due to information of concern, the student may be unable to fulfill essential program requirements and could necessitate a withdrawal or dismissal from their academic program. Students enrolled in health colleges and programs have a continuing duty to disclose any arrest, conviction, guilty or no contest plea, or participation in a pretrial diversion program, or its equivalent, for any criminal offense. Students in NSU’s health college programs are required to notify their college dean’s office within 10 days of any arrest, pending criminal charges, subsequent conviction, guilty, or no contest plea, or participation in a pretrial diversion program, or its equivalent, for any criminal offense. Students, other than those enrolled in health college programs within NSU, must notify the student affairs administrator in their respective college—or designee—of said information. A failure to timely disclose any information required above, including arrests or pending criminal charges, may result in disciplinary action, up to, and including, dismissal from NSU. Health Forms Requirements Certificate of Physical Examination Most programs require students to have a certificate of physical examination completed by their physician. Forms will be provided to each matriculant as part of the admissions package or can be downloaded from nova.edu/smc. Students may request that the NSU Student Medical Center perform these examinations. The University Health Service will make appointments in as timely a manner as possible. The appointments, once made, become an obligation of the student and must be kept. These certificates (whether done privately or by the university) will be placed in an appropriate site. Immunization Requirements Students must complete a mandatory immunization form, which must be signed by a licensed healthcare provider. The form can be found at nova.edu/smc. Students in the health colleges may be required to upload proof of immunizations to multiple online portals to satisfy the requirements of their programs and the training facilities where they are assigned. The following immunizations/vaccinations are required of students in health college programs, based on the current Centers for Disease Control and Prevention (CDC) recommendations for health- care personnel:
