STUDENT HEALTH INSURANCE. All full-time students are required to have comprehensive health insurance coverage. As a student, you will automatically be enrolled in the NSU Student Health Insurance Plan, and your student account will be charged. If you are insured under another insurance plan, you must opt out of the NSU Student Health Insurance Plan by completing the online waiver (link on next page) . If your waiver is denied and you believe your health insurance coverage meets the waiver requirements, you may submit an appeal by emailing studenthealth@nova.edu . Include your name, NSU ID (N#) and your NSU student email ( @mynsu.nova.edu ) . In order to opt-out of the NSU Student Health Insurance Plan, your insurance must meet the criteria listed below and on the next page: 1. The claims administrator of your insurance plan must be based in the United States with a U.S. telephone number and address for submission of claims 2. The policy is not a traveling policy 3. The plan provides both emergency and non-emergency health care and mental-health benefits within the local NSU campus area 4. The plan provides inpatient and outpatient mental-health care and chemical dependency benefits comparable to the coverage provided by the NSU Student Insurance Plan 5. The plan provides coverage for prescription medication 6. The lifetime benefit is unlimited 2020-21 STUDENT INSURANCE WAIVER ACCESS The online student insurance waiver for the 2020-21 academic year is now open. Once you have registered for classes and the student health insurance charge has been applied to your student account, you will be able to access the online waiver. The deadline to complete the waiver is September 1, 2020. You will use your SharkLink user name and password to log in to the waiver form at nova.edu/ bursar/health-insurance/waiver-information . HOW TO OBTAIN AN INSURANCE ID CARD Insurance cards will not automatically be mailed. Students enrolled in the NSU Student Health Insurance Plan may print a paper insurance card by following the steps below: 1. Visit Aetna Student Health ( aetnastudenthealth.com ) 2. Type only the numeric portion of your NSU ID # (without the “N”) and your date-of-birth 3. Click on “Submit,” and your ID card will appear below. You can save the image or print your ID card You may also request to have a card mailed to you by calling Aetna Member Services at (877) 480-4161. 8
