GEC Training Evaluations

Please use the digital forms found on this page to document any training you have attended or any presentation you have given. By entering information into the digital forms, the data will be uploaded directly to our database.

If, for some reason, you are unable to use the digital forms above, PDFs of the forms are available below for printing and manual completion.  If manual forms are used, please return the completed forms to Kristine Freitas at 3446 South University Drive, Fort Lauderdale, FL 33328 or via interoffice mail at 3446 UPP.

Completing these forms is important as the information will help us document GREAT GEC's activities in order to produce accurate reports required by the Health Resources and Services Administration (HRSA).

The Community Collaboration Database

The Community Collaboration Database (CCD) is a way for NSU to keep records of education, service, grant and research projects done in collaboration with community partners.  Thanks to your hard work, NSU is one of 115 U.S. colleges and universities in the country that has earned The Carnegie Foundation for the Advancement of Teaching’s 2010 Community Engagement Classification. The classification acknowledges significant commitment to and demonstration of community engagement, and faculty and staff participating in these community engagement activities should be commended for their efforts in serving the public.

The template below can be used for gathering the data for input into the CCD. All required fields are marked. There are certain fields such as “participants served” which will require you to enter a number. If you don’t have the exact count, please enter a conservative approximation.

The form can be completed online, printed and sent to Kristine Freitas (3446 South University Drive, Fort Lauderdale, FL 33328 or via interoffice mail at 3446 UPP) for input into the database. Thank you in advance for your commitment to community engagement and the timely reporting of your activities.
