IT Project Intake Form

This form is to be used by all for requesting a project to be presented to the PMO/IT Steering Committee. Qualifiers for an idea to be considered a project require the following elements:

  • An approved Budget
  • IT component
  • A project sponsor (A project sponsor provides clear support for this project or initiative)
  • Not to be considered business usual
