Forms

Listed below are the internal forms used for awards managed by the Office of Grants and Contracts. Please work with the OGC Grant Officer assigned to your college/center/department to determine which forms are required for your award; Grant Officer assignments are accessible through the "Staff" link on the left side of the screen. All forms are provided in both Adobe Acrobat and Microsoft Word versions.

Acrobat - if you would like to print a form and complete it by hand or complete a form online and immediately print a copy, please use the Adobe Acrobat version.

Word - if you would like the ability to begin completing a form on your computer and then save it to finish later, or email it to someone else to complete, please use the Microsoft Word version.

All forms must be signed, preferably in blue ink, before submission to the Office of Grants and Contracts; please obtain any and all other required signatures (e.g., dean, chancellor, etc.) before submission. Please contact your assigned Grant Officer with any questions about how to complete your required forms.
