• Benefits Enrollment Notice (for benefits-eligible staff working at least 28 hours/week)

• Child Abuse Reporting Acknowledgement
• Child Discipline Policy
• Conflict of Interest Statement
• Emergency Contact Information
• Employee Release of Information Waiver
• Employment Reference Check (fill out Section I only)
• (I-9) Employee Eligibility Verification (fill out Section I only and bring original ID as outlined on last page of document)
• Managed Care Employee Acknowledgment
• Orientation Agreement Form
• Parking Permit Application
• Payroll Direct Deposit Authorization
• Photo Consent Form
• Physician’s Statement of Good Health (to be completed by licensed physician)
• Tax-deferred Annuity Program
• Transcript Request and Transmittal (official transcript required for exempt-level positions; otherwise photocopy of highest degree)
• Tuberculosis Screening Form (to be completed by licensed physician)
• Security Background Check
• W-4 Form
