NSU's Annual Open Enrollment

Open Enrollment begins Monday, February 11, 2019 and will end at midnight on Friday, February 22, 2019 for the plan year beginning April 1, 2019. Open Enrollment is your opportunity to freely make benefit election changes, add or remove dependents, and re-elect Flexible Spending Account elections.


Forgot your ICUBA Portal password? If so, call 866-524-5036 for password reset assistance, Monday through Friday between the hours of 8:00 am and 6:00 pm CST. Be sure to mention you are from an ICUBA school!

Login to the ICUBA Portal for single sign on access to your benefits - 24 hours a day, 7 days a week!

Login to the ICUBA Portal now and review your current elections, familiarize yourself with Ask Emma, and review your address and other personal information. 

Register for an Online Open Enrollment Session via GoToTraining now! Sessions are scheduled throughout Open Enrollment and many sessions are scheduled after 5:00 pm – in case you are unable to attend during regular business hours

Two Open Enrollment Fairs are scheduled on Wednesday, February 20, 2019, and Thursday, February 21, 2019, from 8:30 am until 5:00 pm in University Park Plaza North on the Fort Lauderdale Campus. More information will be sent via email, so please monitor your SharkMail!

Medical Insurance Plans from BlueCross BlueShield

HRA contributions are now the same for each medical insurance plan at NSU!

Employee Only elections in the Premier, Preferred, and $4K/$8K PPO plans will receive $50.00 at the end of each month in their Health Reimbursement Account.

Employee plus Spouse elections in the Premier, Preferred, and $4K/$8K PPO plans will receive $100.00 at the end of each month in their Health Reimbursement Account.

Employee plus Child(ren) elections in the Premier, Preferred, and $4K/$8K PPO plans will receive $100.00 at the end of each month in their Health Reimbursement Account.

Family elections in the Premier, Preferred, and $4K/$8K PPO plans will receive $100.00 at the end of each month in their Health Reimbursement Account.

Premier PPO Summary Effective April 1, 2019

Employee Only coverage is $243.00 per month

Employee + Spouse coverage is $944.00 per month

Employee + Child(ren) coverage is $696.00 per month

Family coverage is $1206.00 per month

Dual Enrolled Family coverage is $939.00 per month*

*Dual enrolled employees are defined as two married NSU employees who cover eligible dependent children on a medical insurance plan.

Preferred PPO Summary Effective April 1, 2019

Employee Only coverage is $99.00 per month

Employee + Spouse coverage is $681.00 per month

Employee + Child(ren) coverage is $437.00 per month

Family coverage is $863.00 per month

*Dual Enrolled Family coverage is $536.00 per month*

$4,000/$8,000 Deductible PPO Summary Effective April 1, 2019

Employee Only coverage is $94.00 per month

Employee + Spouse coverage is $674.00 per month

Employee + Child(ren) coverage is $241.00 per month

Family coverage is $644.00 per month

*Dual Enrolled Family coverage is $335.00 per month*

*Dual enrolled employees are defined as two married NSU employees who cover eligible dependent children on a medical insurance plan.

The deductible and out-of-pocket maximum limit for the Premier and Preferred PPO Plans will be increasing by $500.00 effective April 1, 2019. There are no changes to the $4K/$8K Deductible PPO plan

Your annual  deductible does not apply to any of the services listed in the table below. Some of the services may be covered at 100% when performed by an appropriate in-network provider, please refer to your plan documents for detailed information about covered services.

Effective April 1, 2019, employees and their dependents who are enrolled in a medical insurance plan at NSU will have automatic access to SurgeryPlus+ for non-emergency, elective procedures like knee and hip replacement surgeries. All procedures must be scheduled directly with a Surgery Plus+ Concierge Care Specialist who will provide you with a list of 3-5 surgeons to choose from.  

The best part?  When eligible procedures are scheduled through a SurgeryPlus+ Concierge Care Specialist, you don't pay!  You read that right...no deductible and no out-of-pocket maximum apply. You may even be eligible for travel reimbursement if you elect to use a surgeon more than 50 miles from your home.

Gap Insurance from American Public Life

Gap insurance can help offset medical expenses you may incur before you meet your deductible and out-of-pocket maximum for the new plan year.

In order to enroll in Gap insurance during Open Enrollment, you must be simultaneously covered under an NSU medical insurance plan at NSU. Gap insurance plans have the same tiers as the NSU medical insurance plans - employee only, employee+child(ren), employee+spouse, and family.

Prescription Pharmacy Benefits from OptumRx

Prescription Pharmacy Benefits are automatically attached to your medical insurance from BlueCross BlueShield, the Rx copays are the same for all medical plans available at NSU.

Pharmacy copays listed below are for a 30 day supply, prices differ for 90 day supply and mail order.

Preferred Generic Drugs: $0.00 copay at the NSU Pharmacy

Preferred Generic Drugs: $5.00 copay

Generic Drugs: $10.00 copay

Preferred Brand Name Drugs: $40.00 copay

Brand Name Drugs: $75.00 copay

Specialty Pharmacy Drugs $75.00 copay

*Specialty Pharmacy Drugs must be coordinated through Briova.

Dental Insurance Plans from Humana and Vision Coverage from EyeMed

Humana's DHMO CS250 is a dental HMO available to participants in the state of Florida only with an $1,800 maximum orthodontia benefit per covered member.

Humana's Preventive Plus PPO is a PPO plan that provides in and out of network benefits for basic and preventive services, no major services are covered.

Humana's High Coverage PPO is a PPO plan that provides in and out of network benefits for preventive, basic, and major services with a $2,000 maximum orthodontia benefit per covered member.

The dental HMO plan from Humana is available to participants in the state of Florida only.

Employee premium is $11.83 per month

Employee plus One Dependent premium is $23.73 per month

Family premium is $36.85 per month

The Preventive Plus PPO plan from Humana provides coverage in and out of network, no major services covered.

Employee premium is $19.67 per month

Employee plus One Dependent premium is $45.72 per month

Family premium is $75.69 per month

The High Coverage PPO plan from Humana provides coverage for preventive, basic, and major services in and out of network, with a $2,000 orthodontia maximum per covered member.

Vision insurance for prescription glasses and contact lenses is available from EyeMed*. The Base plan provides new frames every 24 months, the Enhanced plan provides new frames every 12 months.

*EyeMed coverage is for routine optometric exams, glasses, and contact lenses. Care for the health of your eyes is covered under your medical insurance plan.

Pet Coverage from Pet Benefit Solutions

You asked and we listened!

Effective April 1, 2019, employees will be able to elect the Pet Assure with Pet Plus Rx benefit from Pet Benefit Solutions for single and multiple pet households.

Pet Assure provides discounts on all in-house medical services with no deductible, no exclusions, and no waiting period. Pet Plus prescription coverage provides access to reduced cost prescription medication with free shipping or pick up at more than 50,000 pharmacies. Coverage is available for single and multiple pet households.

Flexible Spending Account Election Reminder

If you wish to maintain Flexible Spending during the new plan year, you are required to make a new election during Open Enrollment. Changes requested after Open Enrollment closes on February 22, 2019 will not be permitted unless you experience a qualifying life event as defined by Section 125 of the Internal Revenue Code.

If you choose not re-elect Flexible Spending, you are able to pay for eligible expenses incurred through June 15, 2019, but your flex election will end on March 31, 2019 and your last deduction will be taken from your pay check dated March 15, 2019. 

Health Reimbursement Account (HRA) contributions provided by NSU do not expire and do not have to be re-elected.
