Orientation Attendance Questionnaire
It is understood that all members of the 2009 Entering Class will attend the functions listed in the Orientation Day Schedule. Family members and significant others are also invited to attend.
This includes the Opening Ceremony and breakfast or luncheon, as well as the informational sessions and tours that follow. Please indicate the number of people in your party (immediate family including yourself) who will attend these events:
Orientation for students entering the Vascular Sonography, Audiology, and Nursing programs will be held on the following dates:
