Allied Health and Nursing

Welcome to the Nova Southeastern University Health Professions Division and Congratulations on your acceptance into the College of Allied Health and Nursing. You are entering a new and exciting phase of your education.

In order to prepare for the new year ahead, please read this website and become familiar with the information we are supplying. In addition, make a note of the required forms that you will need to submit prior to registration day.

The following forms must be printed, filled in, and returned no later than Orientation Day, unless otherwise noted:

Orientation Attendance Questionnaire

Orientation for students entering the Vascular Sonography, Audiology, and Nursing programs will be held on the following dates:

RN to BSN Nursing Program Tuesday, August 25, 2009 4:00pm

Please fill out the Orientation Attendance Questionnaire below no later than August 21st to confirm your attendance.

Orientation for the Entry Level Nursing Program at the Ft. Myers site will be held on Tuesday, September 1, 2009. All incoming students are required to attend. Additional information regarding this program will be forthcoming.

Attendance and Dress Code

All students must sign and abide by the attached Attendance and Dress Code Policy. If you have not already turned one in, please print it and bring it with you to Orientation.

Florida Residency

The following policy on Florida Resident Tuition must be filled out no later than Tuesday, September 1, 2009. Please note that Audiology and Vascular Sonography students do not need to fill in the Florida Residency forms. Nursing students in all programs are required to fill in the Florida Residency forms.

Background Checks

Accepted applicants and students are required to authorize the Health Professions Division to obtain background checks. Information regarding this procedure and a pin number to be used to access the appropriate website is sent to our incoming students after they have contacted us to let us know that they are definitely planning to attend Nova Southeastern University. If you have not received your program code and background check pin number, please contact the Office of Admissions at 1-800-356-0026, Ext. 1101 or (954) 262-1101. Please note that incoming students must complete the online background check documentation within ten days of the receipt of their pin number. If you have any difficulty downloading your information, please contact Certified Background at (888) 666-7788.

Hospitalization Insurance Information

The Health Professions Division is concerned for the health and welfare of its students. As a result, all fulltime Health Professions Division students at Nova Southeastern University are required to carry adequate health insurance coverage. Therefore, as of June 1st, all fulltime students will be automatically billed and enrolled in the Blue Cross Blue Shield plan endorsed by NSU. For detailed information on the plan, benefits, and cost please visit our website www.nova.edu/smc (click on health insurance).

If you carry your own insurance, you will be required to complete an online waiver that can be found on the above website. If you fail to complete the online waiver by the given deadline, your fees and enrollment will remain. Please note that you will not be able to complete the insurance waiver until after you have registered for classes (at Orientation).

Please note that the online waiver is the only way you will be able to cancel the student health insurance plan.

Mandatory Immunization and Physical Exam

All incoming students must have a thorough physical examination prior to matriculation. Please print out the mandatory immunization and physical examination forms located in the right hand side bar and bring them to your physician to fill out. Once your physician has completed these forms, please scan them into your computer so that you can upload them on the Certified Background website. To access this site, please go to www.CertifiedBackground.com and click on “Students”. You will be prompted on how to proceed. If you have any difficulty with this procedure, or do not have the capability to scan and save documents, you will be given the option to send copies of the completed documents directly to Certified Background. Questions regarding the mandatory immunization and physical examination forms can be directed to Certified Background at (888) 666-7788.

Please note: DO NOT SEND ORIGINAL DOCUMENTS -– YOU MUST KEEP A COPY FOR YOUR RECORDS! If you must send the forms to Certified Background, please send them via certified mail.

Lab Coat Order Form

All HPD students enrolled in on-campus programs are required to purchase two lab coats. Orders can be placed on line by clicking on the following link: www.webmedbooks.com/nova/default.aspx. Any questions regarding the ordering process can be directed to the NSU Bookstore, 954-262-4750.

NSU Email Account

Students may obtain their email account credentials by visiting Get My SharkLink ID.

Parking Permit Application

All students, faculty and staff at Nova Southeastern University are required to obtain a parking permit. Please go to the NSU Public Safety Website and print, fill out, and return the Parking Permit Application.

This form must be returned no later than Orientation Day.

NSU Sharkcard

The NSU Card is the official Nova Southeastern University identification card. All students, faculty, and staff are required to carry the NSU Card for identification purposes. In addition, the card can be used as a library card, for vending machine and copy machine purchases, for meal plans, and for building and parking access on campus.

Please fill out the NSU Card Agreement Form completely and bring it with you to Orientation in order to have your photo ID taken. The NSU Card Office will keep this form on file in the office. Should you require a copy of the form please have it made prior to turning it in.
