Congratulations on your upcoming graduation from Nova Southeastern University. The Health Professions Division is making available to all graduates ten (10) personalized graduation announcements at no charge. The announcement will include your name and degree. In order to receive your free announcements, you must submit this form and print a copy for your records. Students graduating from the Colleges of Osteopathic Medicine, Pharmacy, Optometry, Medical Sciences and Dental Medicine must return this form by April 15th. Students in the College of Allied Health and Nursing must submit this form by July 15th.

For your graduation announcement we will need your full name, as you would like it to appear on your announcement, as well as on your diploma. Include degree. Please indicate below.
