To receive your plate complete the following instructions:

• Click here to download the plate Revenue Collection Division affidavit form (this is a fillable PDF form).
• Fill-out the highlighted areas
• Print the form
  
• Registered owner must sign to complete the form
  
• Supply a copy of registered owner's driver's license
• Supply a copy of vehicle's current registration
